In this form, the fields must be filled including the name of employee,  the date notified, taken by, employee number or social security number, date of demise, school/ office, telephone number, contact person, and some more fields.

  • Lump Sum Vacation Form

Are you  previously assigned to an A-basis assignment and/or changed to a non-vacation earning position? If so, you may request in writing payment of your accumulated vacation. If there is no request, a lump sum vacation payment will be made at the end of the school year following the school year where you ceased to get vacation. If you want to request a lump sum vacation payment, this form must be completed including your employee number, first name, last name, email address, home or cell number, MI, work number, and assignment change date.

  • Replacement Warrant Request Form – This form is for replacing a lost or missing check

In this form, make sure that you fill out the fields including your first name, MI, last name, employee number, warrant number, state, ZIP code, address of payee, issue date, net amount, reason for request, and some more fields.

  • W-2 Inquiry Form

In this form, the fields such as the employee name, employee number, checking the box number in question, reason for inquiry, home address, city and state ZIP, telephone number, email address, signature and date must be filled. If you have completed the form, next you must fax it to 866-761-7413.

  • W-2 Duplicate Form – This form is for requesting a duplicate 2020 W-2 Statement
